In order to get access to you FBI criminal history report, you will need to submit a set of fingerprints to the FBI.

Requirements for FD-258 Fingerprinting Cards

Before fingerprinting, you must obtain the standard fingerprint form (FBI FD-258 cards). Any cards previously processed cannot be accepted. All finger print cards must be current, unused, and as recent as possible. All cards must have your name and date of birth on them.

The FD-258 cards must include rolled and flat impressions of all 10 fingerprints, and they must be administered at the same time. All FBI fingerprinting services must be administered by a fingerprinting technician through agencies such as Atlanta Fingerprinting.

Fingerprints must be hardcopies, and if the print is not legible, then they will be rejected. Any rejection could cause delays in processing and will result in additional fees. Trained technicians can help you avoid errors and can fix mistakes that are made.

The name on your response letter must exactly match the name on the fingerprint card, when your completed application, payment, and finger print cards are submitted via mail to the FBI.

Also, note that if the last four digits of your social security number are needed in your response letter, then the full nine digits or last four digits of your social security will be required on the FD-258 cards submitted via mail.
