How to Submit a Fingerprint to the FBI for a Criminal History Report

When submitting a request to obtain your Identity History Summary, you must submit fingerprinting cards, either digitally or by ink.

There are several steps to complete when requesting your Identity History Summary directly from the FBI via mail.

  1. Completing the Applicant Information Form
  2. Obtaining a set of your fingerprints via Atlanta Fingerprinting
  3. Submitting payment through their Credit Card Payment Form, obtaining a money order, or a certified check for $18. (Note: cash, personal checks, and business checks will not be accepted by the FBI, and sending them will delay your processing request and potentially lead to an addition of fees).

For any additional assistance, refer to the Identity History Summary Checklist to review the documents and FD-258 fingerprint card information and to ensure you have included all that you need in your request for a Criminal History Report.

From there, you will mail the required items to:

FBI CJIS Division – Summary Request

1000 Custer Hollow Road

Clarksburg, WV 26306

You may also submit your request to an FBI-approved Channeler. To find the best Channeler for you, our office can give you vetted list of experienced Channelers.
